

She was born July 2, 1929 in Glasford to George and Grace (Tjarks) Melz. She married Jack E. Carlson on June 25, 1949 in Hanna City. He passed away on Dec. 10, 1998 in Peoria. She was also preceded in death by her parents, one sister, Anna Yarnell, three brothers: Harold Melz, Dean Melz, Robert Melz, and one grandson, Matthew Stevenson.
Survivors include five children: Debbie (Tom) Stevenson of Dunlap, Becky Bower of York, PA, Rick (Sheila) Carlson of Greenwood, IN, Bob (Desiree) Carlson of Peoria, Betsy Carlson of Peoria, ten grandchildren and twenty-three great-grandchildren.
Hazel first worked as a Credit Manager for B&M Clothing Store from 1947 to 1951. She later was a Salesperson at Carson Pirie Scott for 12 years from 1973 to 1985 then at Famous Barr for ten years, retiring in 1996.
She was a member of the White Chapel Church in Glasford. She also enjoyed bowling on the Mt. Hawley Bowling League, gardening, and spending time with her family.
Cremation rites have been accorded. A Celebration of Hazel’s life will be Saturday, July 2, 2016 at 2 p.m. at the Davison-Fulton Woolsey-Wilton Funeral Home in Peoria. Visitation will be one hour prior to the services.
Entombment will be in Swan Lake Memory Gardens at a later date.
Memorials may be made to the Arthritis Foundation, American Cancer Society, American Diabetes Association, or the Matthew T. Stevenson Scholarship Trust.
