Samuel Steven Smith passed away unexpectedly from a sudden accident in Phoenix on January 20, 2017. Samuel was 55 years old and went to Cortez High School, became a CNA and was a fanatic sports fan. He has three children .... Christopher Smith, Joshua Smith and Jennifer Smith, and six grandchildren. A Celebration of Life will be held on Monday, January 30, 2017 from 10:00 to 11:00 for visitation and 11:00 A.M. for the Funeral Service at Phoenix Memorial Park and Mortuary, 200 W.
