Born in Providence, he was the son of the late Betty Ann (Myers) Whitehead.
David was the father of Sierra Kelley and her husband Zachary Washburn of Suffield, CT; grandfather of Maxwell Washburn; brother of George Johnson, Jr., Alan Johnson and Brian Johnson and nephew of Patricia McCahey and Judith Kartus.
His funeral services will be private.
