She was predeceased by her husband Kiyoshi Shimizu, son Victor and his wife Laurie. She is survived by her daughters Julianne (Tom) and Lena (Dale), son Ronald (Susanna) granddaughters Kelly Ellis (David), Carlyn (Shea) and Catrina Oliveira, brother Shigeru Uyeyama and sister Harumi Kishi.
We thank the staff at Czorny Alzheimer's Centre for their care and support of Chizuko during her stay. There will be no service and koden is gratefully declined.
She will be forever in our hearts and greatly missed by family and friends.
