He was born October 19, 1956, a son of Jack Baker and Cynthia (Grubb) Sisk.
In addition to his mother, he survived by a sister, Theresa (Sisk) Dalrymple and husband Keith of Texas.
Jack also leaves a number nieces, nephews and cousins.
Follow cremation all services will be private.
