He passed away on Monday, February 23, 2015 in Arlington Heights.John is survived by his wife of 57 years, Tessie (nee Ostapuk); his sons, Jeffrey Slivka and Robert Slivka; his brother, Thomas Slivka; his brother-in-law, Nicholas (the late Lydia) Ostapuk; and his nephew and nieces.He was preceded in death by his parents and his brothers, Michael (the late Lauretta) Slivka and George (the late Connie) Slivka.Visitation from 9:00 am until the time of services at 10:30 am, Saturday, February 28, 2015 at Holy Resurrection Orthodox Church, 1449 N.
