Tyler was a 2010 graduate of Chain of Lakes Collegiate High School.
He is survived by his mother Donna Prochaska, step-father John Prochaska, grandfather Donald Caraway, step-brother Corey Prochaska (Stephanie Siders), nephew Liam Prochaska, niece Kallie Prochaska, and step-grandfather Gerald Prochaska (Joyce). He was preceded in death by his step-brother Cody Prochaska, grandmother, Barbara Caraway, great grandmother Susie Elingburg, great grandfather Edward Caraway, great grandmother Margenia Caraway, and step grandmother Mary Prochaska.
