Jeff was born on November 6, 1979 in Grand Saline, Texas, and died on November 9, 2012 in Eugene, Oregon.
He was a special gift to his parents, Clint and Pat Maddox, who adopted him on his first birthday. Jeff and his brother Steven shared many happy times together. He will be dearly missed by his family and friends.
