Born on May 16, 1934 in Antigo, WI, son of Orville and Minnie Schmidt Polzin. He married Janet S. Folkerts McKinley in 1981.
Surviving are his wife, Janet of Bloomington; children, Jeffrey (Shawn) Polzin of Bloomington, Michael Polzin of Murfreesboro, TN and Melissa (Christopher Frame) McKinley of Lake Tomahawk, WI; five grandchildren and one great-grandchild.
He was preceded in death by his parents; a daughter, Debra Polzin and three brothers, Orval, Earl and Jerry Polzin.
Dave was a United States Army Reserves veteran.
He worked for more than 40 years at State Farm Insurance Company before his retirement as head of the State Farm
Foundation. In retirement he loved spending his winters at the beach with Janet at their condo and traveling to see family and friends.
Per David's request, there will be no visitation or service. Cremation rites have been accorded by East Lawn Funeral Home, Bloomington. A private entombment will take place at East Lawn Memorial Gardens Mausoleum at a later date.
In lieu of flowers, memorials may be directed to the Home Sweet Home Ministries.
