Mollie Form

Mrs. Form, 94, passed away June 9, 2017. She was born July 5, 1922 in New York City, a daughter of the late Levi and Sarah Liss Greenberg. Mollie was a loving and dedicated wife, mother, mother-in-law and grandmother.

She had an amazing intelligence, a gift for writing and a spunky personality which she shared with the Oasis Club at the Jewish Community Center. Her strong sense of justice led to her membership in the Senior Democrats in Charlotte.

Mollie is predeceased by her husband of 35 years, Irving Form. She leaves behind a daughter, Lori Bellefond and son- in-law Jay; a son, David Form and daughter-in – law Chris; and two grandchildren, Caroline and Aaron Form. She will be missed by all whose lives she touched.

Graveside services will be held on Sunday June 11th at 2:00 PM at the Hebrew Cemetery, 1801 Statesville Ave., Charlotte. Shiva will be held at the home of Lori and Jay Bellefond only; on Sunday, Monday and Tuesday from 6:00-8:00 PM.
