Maciej Adam Mindak was born on June 13, 1978 in Warsaw, Poland to Alina and Romuald. He passed away August 14, 2019 in Chicago, which has been his home since 1992. He is survived by both his parents, step-father Michael, sister Gosia, uncle Waldek (Teresa), aunts Jolanta (Andrzej) and Maria (Wlodzimierz), nieces Gabriela, Liliana, and Emilia, nephews Nicholas, Alek, and Maks, nine cousins (Kasia, Jon, Marianna, Wojtek, Ania, Basia, Piotr, Maja, and Pawel), their spouses and children, as well as countless dear friends, both in the US and in Poland.
Maciej was a man of the outdoors; he loved nature, long walks in the woods, and was especially passionate about fishing. He loved working with his hands and was a talented wood carver. He had a good sense of humor and a unique perspective on life. Maciej was an unforgettable personality, and we are going to remember his uniqueness and smile every time we think of him. We will miss him dearly and carry his memory with us forever.
Funeral services will be held on Tuesday, August 20th, at 9:45am from Malec & Sons Funeral Home, 6000 North Milwaukee Avenue, Chicago, Illinois 60646, to St. Priscilla Catholic Church for a funeral mass at 10:30am.
