He was born in Cleveland, the son of the late Carlo and Giovanna DiSiena Costanzo. Tony is the loving father of Alyssa Chojnacki (Phillip Jr.) and Andrea Kathman (Raleigh); cherished grandfather of Oliver Chojnacki; dear brother of Maddalena Costanzo (Stanley, deceased), Francesco Costanzo and the late Raffaele Costanzo; adored uncle and great-uncle to many; loving companion to Rossanna Reyna.
He bravely fought and ultimately lost his battle with ALS, but never lost his zest for life. A true lover of the outdoors, he enjoyed golfing, hunting, and exploring the beauty of nature. National parks, nature shows, and movies brought him joy and peace.
An avid sportsman, he played sports in school and remained a devoted follower of professional sports, especially football. In his younger days, he was a lifeguard at Edgewater Beach—always drawn to the water and sun.
He shared countless vacations and cherished adventures with Rossanna, creating memories that will last forever. Whether with lifelong friends or beloved family, he lived life to the fullest and left a mark on everyone he met.
One of his greatest joys came later in life: spending time with his first grandchild, Oliver, who brought a light to his eyes and warmth to his heart.
His strength, laughter, and love will never be forgotten.
The family will receive friends on Sunday, July 20, 2025 from 2PM-4PM and 6PM-8PM at Craciun Berry Funeral Home, 7200 Detroit Avenue, Cleveland. A Mass of Christian Burial will be celebrated on Monday at 12PM at Our Lady of Mount Carmel Church, 6928 Detroit Avenue, Cleveland, Ohio 44102.
