Addy Rose Sylvester was born on March 18, 1015. She passed away unexpectedly on Thursday, February 4, 2016. Addy was the beloved daughter of Matthew and Sarah (Hoekzema) Sylvester. She was the dear granddaughter of Beth and Greg Hoekzema and Mike and Lisa Sylvester; adored niece of Seth Horstmeyer and Shannon Collier and Paul (Cathy) Hoekzema; loving cousin of Callan Hoekzema, and leaves behind many loving friends and family. She was preceded in death by Matt’s brother Michael.
Our sweet Addy girl was a bright light, loved by all, who left us too soon.
Visitation will be held Tuesday, February 9, 2016 at 10AM at St. Matthew’s Catholic Church, 807 Havens Corners Rd., Gahanna, with a Mass of Christian burial at 11AM. Private interment will follow. In lieu of flowers, memorial contributions may be made to National Christian Foundation Ohio, 11625 Rainwater Drive, Suite 500, Alpharetta, GA 30009.
