Age 38 of Crystal, passed peacefully with her family at her side on 12/3/2017.
She was preceded in death by grandparents, Ron & Marie; uncle, Paul Perez; and nephew, Isaac Smith.
Tammy is survived by parents, Theresa (Rich) Lieske & Stephen (Mary); sister, Amy (Jeremy) Smith; niece, Makena; nephews, Zachery & Reegan; grandparents, Theresa & Paul Perez; loving partner, Chris Nimtz; numerous aunts, uncles, cousins & friends.
Visitation will be held from 4-7PM, Friday, 12/8/17, with a Service of Remembrance at 7PM, all will be held at Gearhart Funeral Home, 11275 Foley Blvd. NW, Coon Rapids, MN.
A private interment will take place at Epiphany Cemetery.
In lieu of flowers memorials are preferred to: Partnership Resources, Inc., United Cerebral Palsy of MN or Courage Center.
