Donations are being accepted in Michael's name by the Southern California Hospice Foundation; if you would like to make a donation, instructions follow:
1. Checks can be made payable to Southern California Hospice Foundation, 12072 Trask Avenue, #100, Garden Grove, CA 92843. If you would like an acknowledgement sent notifying relatives of the donation, you could enclose a note stating 'please send an acknowledgment to Michael Wolf.
2. You can log into our website at www.socalhospicefoundation.com and click on the "How to Help" tab and then click on ‘online submission’ which will take you to a form where you can fill in credit card information. That form also has a section (near the bottom) where you would indicate that your donation is made "In Memory of Michael Wolf. We have a Special Instructions Section where you could indicate to whom you would like an acknowledgment sent with any special message.
