Jacob Donovan Petriak

Jacob was born Feb. 2, 2012, in Woodstock, to his loving parents Christian and Allison (McPherson) Petriak.

Jacob will be missed greatly by his parents; his sister, Chloe; his twin sister, Isabella; his grandparents, John and Joan McPherson, and Sam and Pam Petriak; his great-grandpa, Francis Topper; his aunt and uncles, Brian McPherson, Wade Petriak, Cyndy (Raffe) Benavides; his cousins, Samantha and Alex Petriak, and Lexie and Morgan Benavides; and his godparents, Jennifer and Gary Desserich.

The family also would like to acknowledge his special aunts, Jennifer Billings, Judy Wotring, Christine Wotring, Rachel Pendry, Amy Billings and Aunt Karna, for the loving care they showed Jacob.

A visitation will be from 4 to 8 p.m. Friday, Aug. 10, at Querhammer & Flagg Funeral Home, 500 W. Terra Cotta Ave., Crystal Lake.

The visitation will continue from 9:30 a.m. until the 10:30 a.m. funeral Mass celebration Saturday, Aug. 11, at St. Mary's Catholic Church, 312 Lincoln Ave., Woodstock.

Interment will be in Windridge Memorial Park.

In lieu of flowers, memorials may be made to IDF SCID Initiative, 40 W. Chesapeake Ave., Suite 308, Towson, MD 21204; or to the family for an education fund for his twin sister.
