Carole Anne Smith

SMITH, Carole A., age 51 of Dayton, Ohio passed away Saturday, June 6, 2015. Carole was born in Kenton, Ohio to Myron and Saundra Jacobs (Overly). She graduated from The Community Hospital School of Nursing, received a BSN from Capital University, and an MSN from Wright State University. Carole was a Clinical Nurse Specialist at Miami Valley Hospital in Dayton, Ohio where she was a two time recipient of Miami Valley Hospital’s employee wall of excellence award. She was a Covenant member at Fairhaven Church. Carole was preceded in death by Merle & June Overly and Pearl Pickering (grandparents) on the maternal side, of Kenton, Ohio and Clarabelle & Raymond L. Jacobs (grandparents) on the paternal side of Kenton, Ohio. She is survived by her parents Myron and Saundra (Overly) Jacobs, husband Steve Smith of 14 years, daughter Heather (Andy) Erickson, granddaughter, Addy of Dayton, Ohio, daughter Courtney Bailey, granddaughter, Alana Collins Kettering, Ohio, daughter, Brenna Smith Blanchester, Ohio, brother, Ron Jacobs, niece, Makenna Jacobs of Delaware, Ohio, brother, Michael Jacobs and Allison Ormischer of Bournemouth, England, brother in law, Matt (Emily) Smith, nephew, Parker Smith, uncle Raymond (Jane) Jacobs, cousins, Karla and Krista Jacobs, and uncle, Robert (Lynn) Jacobs. The family will receive friends from 5 to 8 pm, Thursday, June 11, 2015 at Fairhaven Church 637 E. Whipp Rd, Centerville, OH 45459. Funeral services 10 am Friday, June 12, 2015 at the church.
