FIELDS, Jack A. age 88 of Dayton passed away December 3, 2014 at Hospice of Dayton. He was preceded in death by his wife Peggy Fields, parents Irene and Herman Fields, brother Clark Fields, and mother and father in law Florence and Harry English. Jack is survived by his son Jack Fields Jr., daughters Connie Medcalf and Sheree Fields, granddaughters Nicki and Kylie Fields, and great-grandson Brennen Rivers. A visitation for Jack will be held on Sunday, December 7, 2014 from 6-8 pm at TOBIAS FUNERAL HOME- BELMONT CHAPEL with a funeral service to follow Monday December 8, 2014 at 10 am at the funeral home. Burial will follow services at Woodland Cemetery. In lieu of flowers, the family requests that memorial contributions be made to Hospice of Dayton in memory of Jack Fields.
