Sam was a patient since I purchased my dental office in 1993. We got to know each other very well. He gave me advice about family, work and life. I will miss him very much. My sympathies to his entire family.

Sam it was a pleasure working with you all of these years and I will always remember your "strange" sense of humor, your large heart and ever present smile. You will be missed by many but will always be remembered by them also. All of my prayers are with your family during this emotional time.

Sam will be missed more than words can say. His sense of humor was a gift that brought laughter to all of us at the office and his concern for others was just a part of what made Sam so special to everyone.

Eternal rest grant onto him oh Lord, and let perpetual light shine upon him.
