Fay was born on November 2, 1917 in Appanoose County in Iowa, the daughter of Leland and Ruth Dibble Murdy. She married Vern Glen Jack on December 27, 1952 in Mammoth Springs, Arkansas . He preceded her in death on May 22, 1998.
She is survived by her son, Roger (Flo) Jack in East Peoria; three granddaughters; and one brother, Carroll V. Murdy, of Centerville, Iowa.
Fay was also preceded in death by her parents, two sisters, three brothers, and one son , Byron E. Jack.
She attended Apostolic Pentecostal Church.
Graveside services will be at 11:00 am Monday, November 2, 2015 at Fondulac Township Cemetery. Pastor Dan Capperrune will officiate.
