He was preceded in death by his Parents, Michael and Deborah (Miller) Ballard; He was a member of the First Baptist Church in Dayton, Ohio.
He is survived by his Wife, Lisa Ballard; Daughters, Lashae Goddard and Olivia Ballard; Brother, Donald Ballard; Sister, Yolaanda Ballard; and 6 Grandchildren with one on the way.
Cremation was chosen.
