Dean G. McIntosh, age 51 of Winchester, Kentucky, formerly of Middletown, passed away Sunday, July 6, 2014 at the University of Kentucky Medical Center in Lexington. Dean was born July 31, 1962 in Middletown, Ohio the son of Herman Dean McIntosh and Linda Lou Mynhier McIntosh.
Dean is survived by his wife of 28 years, Lisa McIntosh; daughter, Michelle Poplin of Trenton, Ohio; three grandchildren, Heidi, LuAnne, and Dennis; and a brother, Matthew McIntosh of Middletown.
Preceded in death by his parents.
Graveside services were held at Woodside Cemetery.
