Jack passed away peacefully on Tuesday, January 19, 2016 with his son, Bob at his side. Jack was born in Mount Vernon, KY on October 30, 1935 to Roy and Mattie Cummins. He was preceded in death by his parents and brother Bob and sister Glenna. Jack is survived by his son Bob (Monica) and grandsons Will and Jon, sisters Doris Johnston and Dorothy Coffey and nieces and nephews. Funeral services are Monday, January 25, 2016 at Breitenbach Anderson Funeral Home, 517 S. Sutphin Street Middletown, Ohio at 11:00 a.m. with Reverend Andy Lake officiating. Visitation will be held the same day from 10:00 - 11:00 am at the funeral home.
