John Edward Perry, age 39, of Middletown, Ohio, died Monday December 7, 2015. He was born in Middletown, Ohio on December 16, 1975 to Daniel and Carol (Burke) Perry. He was preceded in death by his brother, Christopher Bryant. John is survived by his mother, Carol Hershey and step father Dale; his father Daniel Perry; two sons, Blake Perry, Kayden Perry; mother of his children, Heather Cooper; good friend, Shawn Foreman; and numerous family and friends. Funeral services are 2pm Thursday December 10, 2015 at Woodhill Cemetery with Pastor Bill Patrick officiating. Burial will be in Woodhill Cemetery.
