Phantha LaVonda Hubbard, age 59, of Middletown, died Friday November 17, 2017 at Atrium Medical Center. She was born in Portsmouth, Ohio on March 3, 1958 to William and Nettie (Tackett) Hubbard. Phantha served in the U.S. Army. She was retired from Otterbein in Lebanon. She was preceded in death by her father. Phantha is survived by two daughters, Skyi Ambrose Hubbard and Marissa Xylina Womacks; her mother, grandchildren, Ember, Gavin, Nadia, Aiden, and William; also three brothers, Berry, Brian, and Jessie Hubbard.
