Sarah Jane Faulkner, age 69, of Middletown, died Wednesday, February 27, 2013 at the Christ Hospital. She was born July 21, 1943 in Campton, Kentucky, the daughter of Howard Moore and Callie Amburgy Moore. Sarah is a member of Oasis Church. She is survived by one daughter, Debbie Victor Perz of Mason; two sons, David Linda Collinsworth of Franklin, John Heather Collinsworth of Middletown; five grandchildren; one great granddaughter, one sister, Alice Moore of Middletown; and three brothers, Rocky Moore and Larry Moore both of Middletown, William Moore of Brown County, Ohio. Funeral services will be 1:00 PM Saturday at the Breitenbach-McCoy-Leffler Funeral Home with Reverend Tim Sheets officiating. Visitation will be from 11:00 AM until service time Saturday at the funeral home. Burial will be at the Butler County Memorial Park.
