She was born November 13, 1934 in Harrisburg, Arkansas to Samuel A. and Savannah M (Holmes) Fooks. Hazel retired from AT&T after 30 years as a Staff Assistant. She was an active member of Northminster Presbyterian Church as well as the Joy Club at the church.
Hazel was preceded in death by her parents; her husband, Donald Hazell; 2 brothers; and 3 sisters.
Survivors include her daughters, Debbie Baldwin, Wendy Hill, Julia (Larry) Kovac, Sandra (Chuck) Schumacher, Cynthia (Robert) Allen; her son, Michael (Sherri Lynn) Hazell; her sister, Betty Cory; her 8 grandchildren; 6 great grandchildren; 2 great-great grandchildren; and many nieces, nephews, and extended family.
In lieu of flowers memorials are suggested to the America Cancer Society.
