Charles David "Chuck" Foreman, 69, passed away peacefully on his birthday which was also Veteran's Day, November 11, 2014. He was born in Claremore Oklahoma and was preceded in death by his parents, Charles and Lewella Foreman and his son, Charles D. Foreman Jr. He was a Navy veteran and served two tours in Vietnam and the South Pacific and was a lifetime member of the VFW. He retired from Banner Health in 2009. He is survived by his wife of 31 years, Judith; sons Clyde (Cynthia), Brad Hawkins and daughter Dawn Larson (Kurt), 11 grandchildren -Jamie (Rico), Katie, Grant, Clyde Jr., Danea, Savanna, Charles David, Jessica, Megan and Nathan, a large extended family and many friends. Services will be held Monday, Nov. 24 th at 11:00 am at National Memorial Cemetery of Arizona on Cave Creek Rd.
