Jan Cook, 83, passed away October 18, 2015. Janice was born in Kennett, Mo. to Silas and Lorine Lane. She was predeceased by her loving husband of 57 years Frank Cook and grandson Travis Cook. She is survived by sons, Brad Cook and Frank(Carol) Cook; grandchildren, Lani Cook, Ashley(Aron) Carrozza, Tyler(Heather) Cook; great grandchildren, Preston Carrozza and Lily Cook; sister, Patricia Caperton; brothers, Burley(Judy) Lane and Charles Lane. Visitation will be held Saturday, October 24, 2015 at 9:00 am at Chapel of the Chimes Mortuary in Glendale, Az. immediately followed by a 10:00 am service.
