Phyllis was born in Cleveland, Ohio on August 17, 1942. She was the second daughter of Edmund Kisielwicz (Kisiel) and Helen Orzech, and the sister of Barbara (Kisiel) Gregg.
She married Jeffery Russel Cichocki, Sr., in 1964 and they had three children together.
Phyllis graduated Magna Cum Laude from Kent State University in Ohio with a B.A. in English and Education and also received a Masters of Public Administration from Cal State University, Long Beach. She worked as a grade school and high school English teacher and later as an administrator for the City of Orange, California.
Phyllis is survived by her children: Jeffery Cichocki, Mitchell Cichocki and Corinn Holloway and her grandchildren: Sophia and Claire Cichocki and Rebecca, Matthew and Shawna Holloway.
