

He is survived by his lovely wife, Carol Vick Polk; daughter, Debi (Steve) Peak; son, Eddy (Renee’) Polk; grandchildren, Morgan (Dustin) Finn, Matt (Abbie) Jenkins, Mitchell Peak, Jaycee Polk and McKenzie Polk; great-grandchildren; Hadden and Westyn Finn.
Jack was a Navy Veteran serving active duty from 1957-1959 and Reserve duty from 1959-1961. He then worked for Vick Brothers Lumber Company; he was Co-Owner of Nashville Wholesale Moulding Company and then retired from Codes Inspector for the City of Goodlettsville.
Jack was a faithful member of Goodlettsville Cumberland Presbyterian Church where he served as a Deacon and Elder.
A Celebration of Jack’s life will be held 2:00 pm Wednesday, December 18, 2013 at Goodlettsville Cumberland Presbyterian Church with Pastor Tim Stutler officiating. Interment will follow in Forest Lawn Memorial Gardens.
Visitation with the family will be 12:00-3:00 and 4:00-8:00 pm Tuesday, December 17, 2013 at the Funeral Home and Wednesday, December 18, 2013 from 12:00 until the 2:00 service time at the Church.
