Survivors include his father, Paul Cyzeski of Palisade; his mother, Terri Hart of Grand Junction; one brother, Thomas Cyzeski Paul of West Hollywood, California; and two sisters, Jackie Hart of Grand Junction, and Jessica Fedorchak of Tampa, Florida.

Jacob “Jake” Cyzeski-Hart, 21, passed away February 11, 2017, at St. Mary’s Hospital. Jake was in a hiking accident that he was not able to overcome.

Jake was surrounded by his parents: father, Paul Cyzeski of Palisade; mother, Terri Hart of Grand Junction; and his sister, Jackie Hart of Grand Junction; along with many of his close friends.

Jake was born August 31, 1995, in Long Beach, California, where he attended Fremont Elementary School. Jake loved the California lifestyle of body surfing, music, and being in the outdoors.

After moving to Colorado in the summer of 2009, Jake made many close friends that shared his passion for music, and for him, particularly the guitar. Jake was a bassist in the band Videl.

Jake is survived by his parents; sisters, Jackie Hart and Jessica (Cyzeski) Fedorchak of Tampa, Florida; and brother, Thomas Cyzeski Paul of West Hollywood, California. Jacob is also survived by his beloved Uncle Jack and Aunt Becky Cyzeski of Olean, NY; Aunt Joan & Uncle Ronald Jurewicz of Olean, NY; along with many cousins, nieces, and nephews.

All friends are invited to A Celebration of Jake’s Life which will take place at The Wine Country Inn, Palisade, CO, on March 11, 2017 at 2:00 PM.
