Hazel Ruth Slezak

Hazel Ruth Slezak, age 77 of Fairfield, OH, passed away Monday, January 19, 2026. She was born in Boyd County, KY. on March 2, 1948, to the union of Burl Johnson and Dorothy Smith. Hazel was employed at Cornerstone Brands, Inc. in West Chester for over 20 years. Prior to that, she was employed at The Juergens Company. Hazel was a very giving and compassionate person who always helped others in need. She enjoyed spending time walking with her friends at her senior living community. Hazel loved her family, playing cards, and occasionally going out to eat and being “the life of the party.”

She was preceded in death by her parents, her loving husband of 42 years, Frederick, and her brothers, Jim, Thomas, and David.

Hazel leaves to cherish her memory, daughter, Angela Slezak (Barcley Holmes); brothers, Lewis and Leslie; sisters, Mildred, Mary, Dolly, Belva, and Vickie; granddaughter, Haley Holmes, as well as a host of other family and friends who will miss her dearly.

Visitation will be held at Rose Hill Funeral Home, 2565 Princeton Rd., Hamilton, OH 45011, on Friday, January 30, 2026, from 10:00am until time of services, beginning at 11:00am. Pastor David Haarmeyer, officiating.
