

He was born April 2, 1956 in Waynesburg, a son of the late Donald E. Sr. and Nancy Presock Forman.
Mr. Forman served in the United States Army during the Vietnam War.
He had worked in coal field construction with RG Johnson and Frontier-Kemper.
Mr. Forman was a car enthusiast, animal lover, a fisherman and hunter, and enjoyed watching old western movies.
Surviving is his fiancée, Nora Rhome of Clarksville; a son Dustin Silfies of Clarksville; a granddaughter, Caelyn Silfies; a sister, Bonnie (Hercy) Willis of Clarksville; a brother, Brian (Pam) Forman of Jefferson; five nieces and nephews, Tony Forman, Jamie Forman, Brigitte Simmons, Dannielle Forman, and Samantha Tuttle.
Deceased is a brother, Ray Forman, and a sister, Marlene Rollins.
Friends will be received from 5 to 8 pm Thursday in the Behm Funeral Homes Inc., 1477 Jefferson Road, Jefferson, PA 15344, Gregory P. Rohanna, supervisor. Private interment will be in Smith Cemetery, Waynesburg with the Rev. Nick Bardo and graveside Military Rites will being accorded by the American Legion Post #330, Waynesburg.
