He was 93 years old.
He is survived by Tomiko Yamagata, his wife of over 67 years; daughters, Gale (Wayne) Evans of Houston, TX and Amy Izutsu of Wailuku; grandchildren, Christopher Evans (Houston, TX), Zachary Evans (Houston, TX), Dr. Mika Yamazaki (Honolulu, HI), and Brent Izutsu (San Jose, CA); great-grandchildren, Kasper and Cowen Evans; brothers, Shigemi Yamagata and Fumiko (Raymond) Iwamoto of Wailuku.
Tsukio was a veteran of the 100th Infantry Battalion. He was awarded a Purple Hear and most recently, a Congressional Gold Medal for his service during World War II. He was also a retired electrician for the State of Hawaii.
A private memorial celebration was held on March 31, and the family requests no koden.
