Survived by daughter Roxie (Gary) and nieces Dorothy (Don), Helen and son Ricky; nephews Timothy-sons Jamie and Alex, Darrin (Leila)-children Logan, Kaley and Nolan, Derek (Robin)-daughter Briar and cousins Marg Riley and Jim (Shirley) Muxworthy. Mike (Elena) Schmidt and Andy Schmidt and family.
Predeceased by husband George Blake; parents Cephas Albert Muxworthy and Harriet Lavenia Cornish; grandparents John and Sarah Muxworthy and Thomas and Amelia Cornish; Uncle Amos and Clarence Muxworthy; sisters Alma (Mervin) Hanson, Vera Muxworthy, Ella Muxworthy and brother Howard Muxworthy; niece Eileen Balogh and nephew-in-law Clyde Reay. Cousins Neil (Alma) Muxworthy, Myrtle Strong and John Muxworthy.
She will also be remembered by a lifetime of treasured friends.
Hazel was a Pioneer Girl leader for 10 years at Christian Missionary Alliance Church. She worked at Western University at UCC in food services for 18.5 years. She was also a member of the Hi Neighbours Ladies Group. Hazel loved to step-dance the Highland Fling and the Sword Dance. She had great faith and prayed for others often. She could be heard singing hymns and cute little songs throughout the nursing home.

A visitation will take place on Thursday October 20th beginning at 5pm at Needham Funeral Service (520 Dundas Street) and ending at 8pm. A funeral service will be held the following day on Friday October 21st, beginning at 10:30am with burial to follow at Maitlandbank Cemetery - 42636 Hydroline Road in Seaforth.
