

Damon Karl "Jack" Hazelip, 83, of Mount Washington, formerly of Louisville, went peacefully to be with his Lord on the evening of August 5, 2016, at his home surrounded by his loving family. He was retired from Exelon, formerly Commonwealth Edison Utility Company, where he had worked for over 36 years.
Jack had served his country in the U S Army in both World War II and the Korean War. He had dedicated his life to his church, Christian Assembly Church, having helped build many of their churches throughout the U S A. He loved going " junk pickin " with his grandsons, and visiting many historical sites. He was a jokester and loved to make his family laugh and smile. Jack was truly a family man, as his family was his life.
He was predeceased by his parents, Damon C. and Lillian D. Bolton Hazelip, and his sister, Doris Bolton Cotton.
Left cherishing his memory is his loving wife of 58 years, Patricia Penrod Hazelip; children, Jeff (Valerie) Hazelip; Doreen (Don) Langley, and David Hazelip; grandchildren, Josh (Jamie) Langley, Andy (Rachel) Hazelip, Katie (Michael) Shults; uncle, Forrest "Forty" Hazelip and many extended family and friends.
A Life Celebration will be held on Thursday, August 11, 2016 at 12 noon at Christian Assembly Church, 9020 Stonestreet Road, Louisville, KY 40272,
with burial in Beth Haven Memorial Gardens. Visitation will be at Arch L. Heady at Resthaven, 4400 Bardstown Road, on Wednesday from 2-8 p.m. Jack will lie in state at the church on Thursday from 11 a.m. until service time.
