Born in South Carolina on May 4, 1946 he was a son of the late Martha McIntosh and Kenneth McIntosh.
In addition to his parents he was preceded in death by two brothers, Arthur McIntosh and Raymond McIntosh; a brother-in-law, Ralph Fitzgerald.
He is survived by his daughter, Dana Lewis and husband James; his granddaughter, Shana Lewis; a great-granddaughter, Rayna James; sisters, Elloree Amos, Carolina Seamster, Linda Burns and husband Randy, Rosa Fitzgerald, and Martha Tweedy and husband Charlie; two brothers, Tony McIntosh and Ronnie McIntosh; and a sister-in-law, Evelyn McIntosh.
