Kourtney Rose-Marie Kozak

Kourtney Rose-Marie Kozak, 14, of Charleston, formerly of Milford, DE. Received her wings at 10:10 P.M on May 15, 2015, and went home to take her place in heaven after a long and brave battle with Spinal Muscular Atrophy. Her loving mother Stephanie Shafer-Bray survives Kourtney and her Stepfather Richard Bray, Jr., as well as her father Wayne Kozak. Her brother Christopher Shafer-Johnson and her sister Emily Johnson, her maternal grand parents Albert and Pamela Rizzo of Lewes, DE, and her step-grandparents Richard and Evelyn Bray of Gilford, NH, also survive her. She is survived by her aunts and uncles Kelly and Robert Mitchell, of VA and Heather and Robert Diamond of DE. Cousins Hannah and Christian Pearson of VA, and numerous great aunts and uncles through out the state of WV also survive Kourtney. She was preceded in death by her grandfather Donald L. Shafer on June 10, 2014, and her great grandmother Garnet M. Shafer. A service to honor the life of Kourtney will be held at 12:00 Noon, Tuesday May 19, at Stevens & Grass Funeral Home, Malden, with her Great Uncle David Workman officiating. After the service there will be a butterfly releasing ceremony celebrating her life. A visitation with family and friends will be held one hour prior to the service at the funeral home. The family would like to thank the nursing staff that has provided such wonderful care of Kourtney though out her 14 years. Especially her devoted nurses Michelle Jarrett, and Brent Wolfe.
