Jacqueline Kozak, née Daum, died peacefully on Friday April 5th at CentraState Medical Center with tender and compassionate hospice care from complications of frontotemporal dementia and motor neuron disease—also known as amyotrophic lateral sclerosis (ALS). During her long illness she had a wonderful live-in caretaker, Lovena Ageiwaa.
Dedicated wife and mother, she was predeceased by her husband of fifty-one years Allen Kozak and is survived by her children Arnold Kozak and Elysa Pizzolato. She is also survived by her older sister Greta Richman.
She was born in New York City in 1942 and lived in New Jersey since 1969. She would have been 77 in May but still—as she had always—looked younger than her age. Formerly a model, she was beautiful and stylish to the very end of her life
She was a talented artist, jewelry maker, and interior designer. She could have made a business from her biscotti and focaccia baking—they were that good. She loved exercise, meditation, and yoga and did all of these until her disease progressed.
A memorial gathering will be held at her home in Monroe on May 18th 2019 at Noon.
