Teresa Lynn Hammond of Phoenix, Arizona age 60 passed away August 6th 2013. She has left behind a daughter Shauna Dale and her husband Kevin Dale as well as 3 grand children Austin Neeley, Nalani Felland and Kylie Felland. Along with sister Jill, brother David, nephews Tony and Danny. She was very loved and will be missed dearly. There will be a private gathering for family only to celebrate her life and cherish the time we had with her. Funeral arrangements are entrusted to Advantage Colonial Chapel 4141 N. 19th Ave. Phoenix, AZ 85015.
