

Harriett Lee Forman Cook, 90, passed away on Saturday, May 5, 2012. She was born August 1, 1921, in Decatur, Texas, to Ray Hugh and Eula Daughetee Forman. After graduating from Decatur High School, she attended Texas State College for Women, now Texas Woman’s University, and graduated in 1942. She worked at Humble Oil Co. and later taught business classes at Robert E. Lee High School, both in Baytown. After moving to Waco she taught at Connally High School until her retirement. She lived in The Woodlands for the last three years.
She was preceded in death by brothers Ray Hugh Forman, Jr., and Joe M. Forman and his wife, La Verne O’Neal Forman.
Survivors include sons Ray Holton Cook and his wife Carolyn of The Woodlands, and Donald Edward Cook of Covington, KY; brother Clint Forman of Waco; sister Eugenia Forman of Denton; sister-in-law, Kathleen Brown Forman of Denton; grandchildren Travis Cook of Atyrau, Kazakhstan, and Allison Cook Milstead and husband, Greg, of Cumming, GA; Hilary Cook of San Marcos and Connie Rodriguez Cook, of Corpus Christi; and great-grandchildren Alyssa, Megan, and Holton Milstead of Cumming, GA; and Isaac and Brianna Secuda of Corpus Christi; and many nieces, nephews, cousins, and friends.
The memorial service will be at 2 pm on Saturday, June 2, at Ted Dickey Funeral Home in Plano, TX, with burial at Plano Mutual Cemetery. Memorials may be made to a charity of donor’s choice.
