David was born to Roy and Helen Butler on August 10, 1940 in Jacksonville, Florida.
He will be greatly missed by his wife of 53 years, Evelyn; son, David "Chip" (Heidi) Butler; daughter, Jodi (Paul) Hage; grandchildren, Ryan, Randy, Rylee, and Robyn Hage, Matthew Lazarus, Heather and Hillary Butler; sisters, Nancy (John) Hardesty, Susan (Nelson) Justice, and many friends and co workers.
The family will recieve friends Friday, July 8, 2011, 6-8pm at Forest Lawn North Funeral Home. Funeral services will be Saturday, July 9, 2011, 11am at the funeral home.
