On Saturday, August 13, 2011 Justin suddenly passed away at the age of 20 years old. He will be sadly missed by his mother Karen, father Tom, sister Larissa, nephew Carter Thomas, grandparents Len & Marlene Kozakewich, his uncle Kevin (Boni) Kozakewich, cousins Mitchell and Sheena, and his girlfriend Kelsey Graham. A Memorial Service for Justin will be held on Friday, August 19 at 2:00PM, at Victoria Avenue Funeral Home Chapel, 2080 East Victoria Avenue, Regina. In lieu of flowers memorial donation can be made in Justin’s name to Children’s Hospital Foundation of Saskatchewan #1-345, 3rd Avenue South, Saskatoon, SK S7K 1M6. Funeral Director Greg Kulak.
