Sam attended high school in Holiday, TX and received a football scholarship to Midwestern University in Wichita Falls, TX.
He was preceded in death by his parents; his first wife, Goldie Mae Swinea Caldwell; his sisters, Winona, Sybil, Bobbie, Joye; and his brother, Troy.
Sam is survived by his wife, Janis Andrews Caldwell; his sons, Steve Caldwell and his wife Paula, Joe Pat Caldwell, MD and his wife Lillian, and Jeff Caldwell, PhD and his wife Diane; stepchildren, LaJana Yalowsky and her husband Russ, Larry Don Andrews and his wife Margaret, and Gavin Andrews and his wife Melva; 6 grandchildren; 5 step-grandchildren; 6 great-grandchildren; 6 step-great-grandchildren; and numerous nephews and nieces.
