Dr. Carolyn R. Kubiak Ph.D.

Dr. Kubiak is survived by her daughter Caryn (Bill) Carmean of Glenarm, two grandchildren: Zane (Kaila) Carmean and Colton (Ane) Carmean, one great-grandchild Cassian Carmean and siblings: Sue Ellen Carroll and Dennis (Linda) Kubiak. She was preceded in death by her son Kent Corine.

Carolyn received her B.A. from Saint Xavier College, M.S and Ed.S. from Southern Illinois University and her Ph.D. From Saint Louis University. Some of her academic positions included: the Director of Pupil Personnel Services, Divernon Unified School District, Administrator at SIU Edwardsville and Administrator and Professor with The California State University System.

Dr. Kubiak’s clinical practices included: Counselor with H. Dean Gray Counseling in Edwardsville, Therapist with Gray Psychological Clinic in St. Charles, MO., Clinical Internship with St. Louis Family Institute at St. Louis University, Clinical Associate with Janet M. Salyers, LMFT, P.A, Tampa, FL. And Private Practice, C.R. Kubiak & Associates, Ph.D., MFCC in San Bernardino, CA. as well as C.R. Kubiak, Ph.D., LMFT, in St. Petersburg, FL.

Cremation rites were accorded.
