He is survived by children and spouses Deborah Formby Burdick, Diane Formby Renger and Jack Renger, Denise Formby Crocker and Allen Crocker. Proceeded in death by wife Doris Jean Formby, son David Formby and grandson Justin Crocker. He is also survived by grandchildren and spouses Michelle Formby, Kim Formby Howell and Zack Howell, Carl Crocker, Patrick and Cassandra Burdick, Melinda Formby and Brandon Bischoff, Sarah Burdick and many great grandchildren. He is also survived by nephew Paul Formby and spouse Mary Beth Formby as well as niece Patrice Johnson and spouse Dr. Paul Johnson. He retired from the City of Stockton Fire Department in 1984. He was a member of the Fire Department for 30 years. Celebration of life will be private. Donations can be made in his memory to Hospice of San Joaquin.
