David Thomas Smith, 67, of Surprise, Arizona, passed away on March 30, 2014. He was born in Onawa, Iowa, on May 14, 1946, to Raymond “Curly” Smith and Edna Mae (Kirk) Smith. He was preceded in death by his parents and brothers, Dean and Terry Smith. He is survived by his loving wife of 44 years, Sharon Smith, daughters, Tammy (Douglas) Simpson and Tracy (Michael) Chentfant, grandchildren, Emily & Grace Simpson and Craig and Natalie Chentfant, brothers, Larry Smith, Bob Smith and Denny (Sue) Smith and sister, Sue Smith. Memorial Services will be held at Rush Funeral Home in Onawa, Iowa. In lieu of flowers, memorial donations may be made to the family.
