Mr. Formby was born May 23rd, 1967 in Texarkana, Arkansas and was preceded in death by his father Millard Formby Jr.
Survivors include his mother Carolyn Formby and two brothers and sister-in-law; Barry Monroe and Millard Formby III and his wife Ann Formby. Also left to cherish his memory is his niece Elizabeth Formby.
Memorial services will be held 2:00 pm Friday, September 25th, 2026 at East Funeral Home-Moores Lane Chapel with Rev. Jaimie Alexander officiating.
