Beloved wife of James Forman; loving mother of Kathy Sweeney-Root; Debra (Jerold) Allein, Sally (David Mangiapane) Chirico, Steven Chalcraft, Catherine (Dennis) Forman-Blattenberger; cherished grandmother of Erik (Twila) Sweeney, Ian Ferdinand, Joshua (Alicia) Ferdinand, John Paul Chirico and Alex Chirico; great-grandmother of six; sister of Paul (Diana) Becker; brother-in-law of Robert (Lorri) Forman and Paul Forman. Also survived by several nieces and nephews. Faye was the daughter of the late George and Eleanor Becker.
